We want your visit to be as comfortable and comprehensive as possible. Depending on the condition, Dr. Attai generally will spend approximately 30-60 minutes with a new patient. All patients are seen on an appointment-only basis, and we make every effort to adhere to our schedule so that the proper amount of time may be devoted to each patient – we ask for your cooperation in this matter.  If you have any questions, do not hesitate to call our office prior to your appointment.

For your convenience, all required paperwork is available below. All new patients should arrive at least 20 minutes prior to their scheduled appointment time to ensure that all paperwork is complete and that all necessary reports have been received.  If you are unable to complete your paperwork ahead of time, please plan to arrive 30 minutes prior to your scheduled appointment time to complete the forms. No one will be seen without their insurance card(s) well as a picture ID – this is for identity protection and is required.

It is extremely important that we have all of your mammograms, ultrasounds, and MRI images (films or CD) in addition to the reports. Dr. Attai is a breast specialist, and review of all reports AND images is part of a comprehensive consultation. If you have previously undergone a biopsy, please bring a copy of the pathology report.  If your images were performed at the Providence St. Joseph Breast Center,  or at any RadNet facility (Encino Breast Care Center, Burbank Imaging Center, SanFernando Interventional Radiology), we will be able to obtain the films and reports – you do not need to bring them with you.

If you do not have this information, your appointment may be rescheduled.
